In this film 16 smoking incidents occured, 11 of which were inside and 5 of which were outside. 

The film was reviewed by Dr. Barry Hummel on 11/7/2007.

Dr. Hummel posted the following additional discussion:  

"In Tim Burton's Batman, most of the smoking scenes are in the background, especially in bars and the newspaper office.

Unfortunately, three prominent characters smoked during the film: Harvey Dent (Billy Dee Williams), the Mayor (Lee Wallace), and Detective Eckhardt (William Hootkins).

The corrupt Detective Eckhardt can be seen smoking a cigar early in the film when he is confronted by reporter Alexander Knox (Robert Wuhl):

More importantly, District Attorney Harvey Dent (Billy Dee Williams) is seen smoking cigars on several occasions.  One time, Dent is one of several characters seen smoking at a fund-raiser at Wayne Manor:

Another time, Dent is seen smoking in his office while trying to track down the Joker:

One positive note:  The director did was able to make the Joker look quite villainous without resorting to tobacco use... a trend continued by Christopher Nolan in The Dark Knight."
